Verification of Certification
IMPORTANT NOTICE REGARDING NBCOT CERTIFICATION AND MISREPRESENTATIONS
Due to a number of certification misrepresentation and misuse matters which have occurred, it is prudent, and therefore highly recommended, that all interested parties contact the NBCOT in order to verify OTC® and/or OT-SC™ certification(s). We encourage interested parties to contact the NBCOT Executive Director, in a timely manner, to determine the current certification status of an individual using a NBCOT credential.
Please note that following receipt of information that an inappropriate or unauthorized use of NBCOT certifications or credentials may have occurred, NBCOT will determine if responsive actions(s) will be taken.
NBCOT is aware that NBCOT certification credentials have been misused and misrepresented with respect to the following individuals:

Verification of an individual's Orthopaedic Technologist Certified (OTC® ) or Orthopaedic Technologist - Surgery Certified (OT-SC™) is available to all interested parties.
As of January 1, 2005, the National Board for Certification of Orthopaedic Technologists will no longer provide verbal/email verifications to Employers or Credentialing Agencies. Please use the official Fax Verification Request Form posted below.
If you wish to obtain an official Certification Verification Letter from the National Board for Certification of Orthopaedic Technologists, this certification information is reported on official NBCOT letterhead and carries the official stamp of the National Board for Certification of Orthopaedic Technologists, the link is provided below.
The fee for an official letter is $30.00. It is sent directly from the NBCOT to the addressee (e.g, regulatory entity, credentialing agency, employer). Payment may be made by group check, bank check, money order, or credit card (Visa, MasterCard Only).
There is a $40.00 fee for any returned check. Requests submitted without the required fee will be returned. Please allow two weeks (2 weeks) for verification and processing.

To be used by credentialing agencies only!
Fax Verification is used to check certificant status only. There is no charge to Medical Staff Offices or Credentialing Agencies for "Status Only Requests". This will include any Disciplinary Actions on file, and general status information.
If you want a Certified, Stamped Letter of Verification, mail this form and send the appropriate payment.
