To process a NAME change request ONLY:
The National Board for Certification of Orthopaedic Technologists requires legal documentation (e.g., marriage certificate, divorce decree or court order) to change certification records. Please submit an original certified copy or a notarized photocopy of your change (i.e., copy the document and have it notarized). Upon receipt of correct change, you may order a new certificate and ID card (see bottom of change form). The cost to have a new certificate and ID card is $15.00 US
